Refund Policy:
Full registration refund, less a $50 handling fee, will be given if notification is received by September 15, 2008; partial refunds are not given. Refund requests should be sent to the above address.

This conference is open only to JASNA members.  You can join as part of the AGM registration process if you aren't already a member.  

The AGM has reached its full capacity. Registrations received after the AGM sold out were placed on a waiting list, but the list is now closed. Members on the waiting list will be notified if cancellations occur.

The AGM Registration fee is US $260 if your form is postmarked on or before July 14, and  US $305 if postmarked after July 14.  The final postmark date to register for the AGM is August 25.

Before completing the AGM Registration Form, please check the AGM Registration Totals table on the left to avoid registering for sold-out events.

Although AGM registration forms were included in the brochure mailed to all JASNA members, we would prefer that you fill out the forms provided in Adobe Acrobat PDF format below.  (Click on the blue AGM Registration Form button below.)

Using this form, you will be able to easily type in your information, and the form will automatically calculate your total fee.  After completing a form, please print two copies, keep one for your records, and mail in the other with your check or money order.  This form must be mailed; it cannot be submitted online.
